Thank you for your support and understanding." as well as provides the information for the Alzheimer's Association, their website and phone number.

This file, provided by our site is a document that we are using for an example, (this link) we made it for our Aunt Margo who has Alzheimer's-Dementia and stayed with us for a month in Las Vegas. I made this document so when she went home, other people caring for her would know what her daily routine is. We are using this link as an example but underneath it, we provide a template.
